Required Information

In order to utilize this Quick Submit app, the following information must be available as all items are required:

Model Number

Serial Number (required for hydrotherapy tubs)

Job Name & Address

Submitter's Name & Phone Number

The Quick Submit app is designed for individual unit claims only. To report damage to multiple units, please submit each claim separately, or click here to call the warranty department for your plant.

Submittal Process

Once all the required information has been collected, open the Quick Submit app and click on the "Submit Claim" tab. You will be asked to provide the model number, serial number, the job name and address, and your name and contact information.  You will also provide the primary reason for the claim. When describing the defect, please be as specific as possible. You may also attach a photo from the photo gallery or take a photo to be attached.
